Research project title: ePREVENT-360 - Empowering Patients. Enhancing PREVENTion

Feasibility of a consumer facing digital intervention to enhance chronic disease prevention in primary care.

We are excited to invite you to join the ePREVENT 360 study, a research project focused on improving the prevention of chronic diseases.

What is involved:

As a participant, you will get to use the In2health digital program, designed to help you to better understand your health and to make positive changes.  

Voluntary participation:

Participation is voluntary. Joining the study is completely up to you. You can leave the study at any time without any need to explain.  

Data privacy:

We take your privacy seriously. If you decide to take part, your data will be stored securely. We will remove your name and any information that could identify you from the data you provide. Your personal details will be kept confidential and will not be shared with anyone outside the research team.  

Project funding:

This study is funded by a grant from the Royal Australian College of General Practitioners Foundation and HCF Research Foundation. It is conducted by a research team from the University of Wollongong School of Health and Society.

The purpose of this research: 

Is to understand whether people are interested in using an online health check (In2health Thrive) to identify their risk of 21 common chronic conditions and to receive a health action plan with personalised health tips including what health screening is recommended. The researchers would like to hear your perspectives on In2health, what you like and what could be improved.  

 

1. I agree to take part in the following

     Online surveys: 

  • Motivation to improve health, understanding of risk and current healthcare access questions (10-15 minutes). 

  • The In2health ‘Health Check’ (20-30 minutes). This can be done in small sections or all at once depending on what is most convenient for you and a 3- and 6-month brief online health check-in (5-10 minutes). 

  • Feedback on the In2health Health Check (7-10 minutes). 

Interview: 

  • After completing the surveys, I may be invited to an interview with a researcher (20-30 minutes). I can accept оr decline this. 

 

2. I understand that

  • There are no expected risks to me from taking part in this research. 

  • My participation in the “ePREVENT-360 study” is entirely voluntary and I can withdraw at any time.  

  • The study will run for 6-months. 

 

3. Data privacy

  • My data will be confidential and used only for this study, not for any future research.  

  • Data collected for this research will be analysed in a large group and published in a PhD thesis, academic journals and presented at conferences. Individual data will not be identifiable.   

  • My personal information will be separated from other research data. All data will be stored securely in a University of Wollongong database for 5 years.  

  • If I agree to take part in an interview, all my personal information will be removed and code names that cannot identify me will be used for research purposes.  

 

4. If I choose not to continue : 

  • My interview data will be removed from the study and will be disposed of.
