Participant (clinician) online consent form

Research project title: ePREVENT-360 research project

Empowering consumers. Enhancing PREVENTion.

A mixed methods pilot feasibility study of a digital chronic condition prevention intervention in Australian general practice.

We are excited to invite you to join the ePREVENT 360 study, a research project focused on improving the prevention of chronic diseases.

What is involved:

Your clinic has agreed to take part in this study, which allows your patients to access a new digital chronic condition prevention tool designed to motivate individuals to choose healthy and to access screening. GPs and nurses will have access to a concise summary of individual risk of 21 conditions and guideline-based recommendations on further investigations and/or lifestyle change.

Voluntary participation:

Participation is voluntary. Joining the study is completely up to you. You can leave the study at any time without any need to explain.

Data privacy:

We take your privacy seriously. If you decide to take part, your data will be stored securely. We will remove your name and any information that could identify you from the data you provide. Your personal details will be kept confidential and will not be shared with anyone outside the research team.

Project funding:

This study is a PhD project that has been funded by a grant from the Royal Australian College of General Practitioners Foundation and HCF Research Foundation grant. It is conducted by a research team from the University of Wollongong School of Health and Society.

We understand how busy general practice is and so will not ask for much of your time. Thank you for considering taking part.

The ePREVENT-360 clinician information sheet and this consent form explains what you will be asked to do if you decide to join this study, the study’s purpose and how we will protect your data.

Research team:

  • PhD Candidate and chief Investigator: Dr Gillian Singleton (GP Researcher) gms993@uow.edu.au

  • Primary supervisor and co-investigator:  Associate Professor Hassan Hosseinzadeh (Public Health Researcher) hassanh@uow.edu.au

  • Co-investigator: Professor Liz Halcomb (Primary Care Nurse Researcher)

  • Co-investigator: Professor Andrew Bonney (GP Researcher)

The purpose of this research:

Is to understand whether consumers are interested in using an online health check to identify their risk of 21 common chronic conditions and to receive a health action plan with personalised health tips including what health screening is recommended.  The researchers would also like to understand how GPs and nurses in busy general practice clinics can be better supported to prevent chronic diseases and so would like to hear your perspectives on what works and what doesn’t. Feedback will also be sought about ways to improve the In2health program in the future.
