Participant (clinic) online consent form

Research project title: ePREVENT-360 research project

Empowering consumers. Enhancing PREVENTion.

A mixed methods pilot feasibility study of a digital chronic condition prevention intervention in Australian general practice.

We invite your clinic to take part in the ePREVENT-360 study. This aims to assess the feasibility of a new digital chronic disease prevention tool, In2health.

What is involved:

Your patients will have access to the In2health ‘Health Check’ designed to identify their risk of 21 chronic conditions and provide personalised action plans with tips on healthy lifestyle choices and screening. GPs and nurses will receive summaries with an aim to reduce workload and improve outcomes. In2health will provide a concise summary of the risk assessment and action plan for your GPs and nurses with an aim to reduce workload, improve outcomes and remuneration.

Voluntary participation:

Participation for both patients and clinicians is voluntary. They can withdraw from the study at any time, no questions asked.

Data privacy:

We take your privacy seriously. If you decide to take part, your data will be stored securely. We will remove your name and any information that could identify you from the data you provide. Your personal details will be kept confidential and will not be shared with anyone outside the research team.

Project funding:

This project is funded by a grant from the Royal Australian College of General Practitioners Foundation and HCF Research Foundation and is conducted by primary care researchers from the University of Wollongong.

Support for this project and any insights that you can provide would be appreciated.

Thank you for considering!

The ePREVENT-360 clinic information sheet and this consent form are provided to explain what your clinic will be asked to do if you decide to be a part of this study and some important information about data privacy.

Research team:

  • PhD Candidate and chief Investigator: Dr Gillian Singleton (GP Researcher) gms993@uow.edu.au

  • Primary supervisor and co-investigator:  Associate Professor Hassan Hosseinzadeh (Public Health Researcher) hassanh@uow.edu.au

  • Co-investigator: Professor Andrew Bonney (GP Researcher)

The purpose of this research:

The purpose of this research is to see whether people are interested in using an online health check to identify if they are at risk of 21 common chronic conditions to obtain a personalised plan about what they can do to improve their health and their healthy life expectancy. Another focus of the research is to identify whether GPs and nurses find the information in the In2health summary useful to improve patient care.

1. Study overview for the clinic:

  • Patients of your clinic aged 30-65 years will be asked to complete a series of online surveys and health assessments over 6 months. A small number of patients who volunteer will be invited to take part in interviews about their experiences (75-105 minutes over 6 months)

  • GPs and nurses will be invited to take part in brief interviews and feedback surveys (10-45 minutes over 6 months depending on participation in interviews).

  • Results of this study will be published in a PhD thesis, academic journals and presented at professional conferences. No individuals or general practice clinics will be identifiable in these publications.

2. Data privacy

  • Patient health data will remain confidential and consists only of information that patients provide through online surveys. No patient information is needed from your clinical records or your clinicians.

  • Clinician data will remain confidential.
